Diagnostics

Patients with acute lung injury may have visible infiltrates on a chest radiograph. Patients with ARDS have a characteristic “whiteout” chest x-ray appearance in which little lung tissue capable of exchanging oxygen is visible. An ABG is useful for determining level of oxygenation.

Intervention

Acute lung injury, especially ARDS, is difficult to manage. Supportive treatment is given, and oxygenation is the primary goal. The use of small tidal volumes has been shown to prevent further lung damage and decrease mortality risk.

NURSING IMPLICATIONS

Maintaining adequate oxygenation is the goal of treatment. The nurse should work with the physician and respiratory therapist to determine the best strategy for maintaining oxygenation in these patients. Usually, this involves the use of low tidal volumes. ABGs are used to determine if treatment is working or if changes need to be made.
